Job Description

JOB SUMMARY

The Director of Community Services will carry the responsibility for the community support services activities in accordance with state regulations. In addition, the Director of Community Services will oversee service delivery by completing assessment screenings, developing individualized care plans, monitoring/observing the work of the Community Support Specialists, and reviewing documentation for quality and clinically-sound practice. RESPONSIBILITIES

  • Participate in the assessment of participants’ service needs, and in the development, implementation, and monitoring of the IRP and/or IPP.
  • Provide supervision of PRP, TCM, and SE services as required in accordance with COMAR and other regulations for community clients.
  • Coordinate and facilitate care meetings and/or groups for clients, addressing the clients’ care needs.
  • Communicate with clinical staff concerning participant progress.
  • Understand and follow directions from the agency administrator, as well as complying with agency/county directives
  • Fiscal year planning and financial responsibilities for the program
  • Maintain accurate, timely records for each case manager.
  • Overview of the spectrum of mental illness, symptomatology, psychotropic medications and the problems presented in adjusting to life cycle changes.
  • Oversee in the supervision of activities, including children’s activities, ensuring that a safe environment is maintained.
  • Provide supervision, consultation, guidance, crisis intervention to assist Community Support and other staff.
  • Provide group supervision, quality improvement and in-service trainings to the PRP counselor program.
  • Perform case intakes by working with referral sources and insurance providers to ensure authorization and verification of PRP, TCM, SE counselors services.
  • Maintain case records in accordance with guidelines under HIPPA and other federal and state Medicaid regulations.
  • Participate in team staffing to review cases within agency to collaborate and maintain standards/certification.
  • Acts as a liaison to other department coordinators and administrators at the agency. Communicate regularly with mental health supervisors to ensure that services are integrated, to problem-solve program issues and to act as a champion for employment. Inform administrators of program successes and challenges at least quarterly.
  • Collects clients and processes outcomes monthly. Shares outcomes for the program, as well as for individual practitioners, with the supported employment team quarterly. Helps team set goals for improvement. Reviews individual outcomes with each practitioner, develops written plans for improvement that include assistance from the supervisor, and helps practitioners set goals.
  • Attending training in areas that relate to the populations served.
  • Perform other duties as required or assigned

REQUIREMENTS

  • State of Maryland Licensed Mental Health Professional (LGPC, LGSW, LCPC, LCSW-C, LCAD) eligible and have 2 years of relevant direct service and/or supervisory experience working with children and adults that have mental health issues.
  • Must exhibit superior knowledge of all PRP, TCM, SE and Medicaid procedures and guidelines.
  • Must have superior knowledge available resources in the service area for people with serious emotional disturbances
  • Must possess the ability to multitask and handle a non-traditional work environment.
  • The ability to be self-started and work independently with limited supervision at times.
  • Experience with Microsoft Word, Excel, Power-point, Outlook Email, and other systems, i.e. electronic time punches, electronic medical records etc.
  • CPR/First Aid Certification must be acquired and maintained once employed.
  • Bilingual in any language a plus.
